Please complete the following form either prior to your first pay per the candidate induction or, if you wish to update your current details.

If you don't have the information required to complete all the relevant fields, please contact your super fund.  Otherwise, we can only make payments to our default fund.

Your membership number
Other identifier (if required by fund)
Full name of your chosen fund
BSB number (if fund accepts EFT payments, 6 digits maximum)
Account number (if fund accepts EFT contributions, 9 digits maximum)
Bpay biller code (if fund accepts Bpay contributions)
Bpay reference number (if fund accepts Bpay contributions)
